As mentioned in the meeting, please use only One collection form on a page. In your case, i am seeing multiple collection forms: https://srt-fleet-solution4267_v1.preview.drapcode.io/driver-data
If you need multiple collection forms, then please use them in Modal popup or on a new page.

Can you please create a new page, drop the fresh collection form, create a new event and then try again?
